Support Coordination

Fundamental to assisting individuals and/or families navigate and coordinate their options for service delivery, as outlined in their NDIS plan is ‘knowing the person’ and seeking to understand their fundamental needs and perspective.

Our framework for support coordination is based on the belief that the development of support options for a person with a disability, needs to consider the broader picture of what is a “good life”. This means contextualising elements that most people consider, equate to a good life including having a purpose, strong relationships and economic security, with what things are important to the person and how they define a good life. We firmly believe that maintaining a positive vision of what is possible will support better outcomes.

At Renew, we are committed to getting to know the person and/or family so that we can facilitate the best options for service provision that will create the outcomes the individual and family are seeking.

NDIS - Disability Services :: Positive Behaviour Support Plans

People who have complex support needs and/or mental health diagnoses can also exhibit challenging behaviours which can cause harm to themselves and/or others. These behaviours of concern more often also create distress and upheaval in home, workplace and community environments. At Renew, we implement a 5 stage model that is person-centred and collaborative in approach. These are:
  • Data gathering and assessment
  • Hypothesis formulation and testing
  • Plan development
  • Training and implementation
  • Review and ongoing follow-up
As we are outcome focused, we work with people in their homes, workplaces and community settings in which the behaviours of concern occur. At Renew, we believe that for a positive behaviour support plan to be successfully implemented, particularly with people with complex needs, requires the time and commitment that is outlined in our comprehensive stages of development.
